Menopause, Weight Gain, and a New Hope for Better Results

Many women notice weight gain during menopause. This happens because of hormonal changes, aging, and shifts in metabolism. Extra weight during midlife can raise the risk of diabetes, heart disease, and other health problems.

New weight-loss medicines are helping, and tirzepatide is one of the most effective options available today. But until recently, we didn’t know much about how well it works specifically for postmenopausal women, or whether menopause hormone therapy (HT) could make a difference.

A new study from the Mayo Clinic Health System looked closely at this question.

What Was the Goal of the Study?

The researchers wanted to know:

  • Does hormone therapy help postmenopausal women lose more weight while taking tirzepatide?

  • Does hormone therapy improve heart and metabolic health along with weight loss?

How Was the Study Done?

  • The study used medical records from postmenopausal women treated with tirzepatide for at least 12 months.

  • All women had overweight or obesity.

  • Some women were using systemic hormone therapy, while others were not.

  • To keep the comparison fair, researchers carefully matched women in both groups based on:

    • Age

    • Body mass index (BMI)

    • Type and age of menopause

    • Diabetes status

    • Past use of weight-loss medications

In total:

  • 40 women using hormone therapy were compared with

  • 80 women not using hormone therapy

What Did the Study Find?

1. Greater Weight Loss With Hormone Therapy

Women who used hormone therapy lost more weight while taking tirzepatide:

  • Hormone therapy group: about 19% body weight loss

  • No hormone therapy group: about 14% body weight loss

More women on hormone therapy also reached major weight-loss milestones:

  • 20% weight loss

  • 25% weight loss

  • Even 30% weight loss

2. Better Heart and Metabolic Health

Both groups showed improvements in:

  • Blood sugar levels

  • Blood pressure

  • Liver enzyme levels

However, women using hormone therapy had extra benefits, including:

  • Lower diastolic blood pressure

  • Lower triglyceride levels

  • Greater improvement in certain liver enzymes

These changes are important because they reduce the risk of heart disease and metabolic problems.

What Do These Results Mean?

This study suggests that hormone therapy may boost the effects of tirzepatide in postmenopausal women. When used together, they may lead to:

  • Greater weight loss

  • Better cardiometabolic health

However, this was a retrospective study, meaning researchers looked back at existing medical records. While the results are promising, they do not prove cause and effect.

What’s Next?

The authors stress the need for:

  • Large, randomized clinical trials

  • Studies that look at how hormone therapy may improve energy balance and metabolism

These future studies will help doctors make clearer treatment decisions for postmenopausal women struggling with weight gain.

Key Takeaway

For postmenopausal women with overweight or obesity, using hormone therapy alongside tirzepatide may lead to greater weight loss and better heart health than using tirzepatide alone. While more research is needed, this study opens the door to more personalized and effective weight-management strategies during midlife.
